information SERVICE DETAILS

Through wellbeing coaching, courses, community groups and supported physical activity, we support people to live well regardless of challenges faced.

We provide self-management support to people adjusting to life with a long-term health condition or facing a challenging life situation. We're also here for people struggling with Long Covid.

Support options include:

- 1:1 Wellbeing coaching
- Support to develop a ‘Stay Well at Home' plan
- Group-based Lifestyle Management courses
- Group-based Mindfulness course
- Physical activity support

At present, we are maintaining a provision of 1:1 telephone support.

We continue to offer support remotely - both online and over the phone.

We know that it can be difficult to adjust to life with a long term condition, and that at the present time many people’s existing supports may not be available, making this even more challenging.

We're also here for people facing the challenge of post-viral fatigue syndrome as a result of COVID-19.
