Learn more about supported self-management

Self-management is about empowering people to take control of their long-term condition, and supporting them every step of the way.

Supporting supported self-management

We regularly work to help people develop practice that better supports people to learn how to self-manage with a long-term condition.

This work develops practice by:

- Introducing practitioners to emerging ideas relating to person centred supported self-management
- Stimulating reflection and conversation between practitioners
- Offering a supportive environment to exchange challenges and share best practice
- Providing information and resources
- Promoting positive changes in practice

A Starter for 10

- Empower people - support self-management in every interaction
- Practice with compassion - practice with care and empathy
- Take a holistic approach - "see the person, not the condition"
- Work in partnership - work collaboratively, valuing the person as the expert on their own circumstances
- Establish personal outcomes - establish what the person hopes to achieve and recognise the value of adopting a future, outcome-focused approach
- Focus on strength - maintain a persistent belief in what's possible for each person and focus on change and small improvements
- Be a facilitator - adopt a facilitative, motivational approach over an authoritative, didactic one
- Connect with local networks - encourage people to make use of community services and self-management groups
- Support risk management - recognise risk taking as part of normal life and recovery
- Invite feedback - learn from feedback after every interaction
